AFFILIATION WILL NOT BE GRANTED UNTIL THE FOLLOWING ARE COMPLETED:
1. All details on this form are completed.
2. A copy of incorporation or a letter of intent to incorporate in 2022 is forwarded.
3. Payment of $100 is made payable to Western Districts Netball Association.

Terms and conditions
- By affiliating with Western Districts Netball Association, I acknowledge that your Club will meet the responsibilities and obligations associated with affiliation, which include:
a. completing an affiliation form, listing all club contact details including management committee members;
b. ensuring that each team can provide an umpire who is competent to umpire at the grade in which the team
participates;
c. ensuring that it can meet its financial obligations to the association in terms of fees and levies;
d. carrying out its rostered duties, as allocated;
e. ensuring that the club is in no way connected to, or affiliated with, any body or association not in good
standing with Netball Queensland;
f. ensuring that all members, when participating in netball with the association, abide by the association's
codes of behavior;
g. maintaining a designated President and Treasurer, as per the affiliation application form;
h. ensuring that the club does not engage in conduct prejudicial to the interests of the association.
